SPECIAL NOTE: Please check your program above and include the initial payment if choosing monthly payment plan. If you do not make a payment and would desire only that a proposal of classes be sent to you, then understand that we must have your official transcripts before we can complete this proposal for you.
*INTERNATIONAL STUDENTS: Due to the extra charge for postal expenses, there is a $175 surcharge for any student with an address outside the United States. All study materials will be sent Registered Air Mail where possible.
REFUND: Should you decide to drop out, you must do so in writing within 30 days after acceptance. All tuition will be refunded except a $200 service charge. Registration and enrollment fees are not refunded. After 30 days there will be no refund except as stated in the catalog.
AGREEMENT: I HEREBY CERTIFY THAT ALL INFORMATION GIVEN HEREIN IS TRUE TO THE BEST OF MY KNOWLEDGE. I FULLY UNDERSTAND THAT ALL ASSIGNED WORK MUST BE SATISFACTORILY COMPLETED AND ALL FEES MUST BE PAID BEFORE I AM ABLE TO RECEIVE A DEGREE OR DIPLOMA FROM COVINGTON. I UNDERSTAND THAT THE SEMINARY WILL SUPPLY ME WITH STUDY GUIDES, BOOKS, IF APPLICABLE, AND FACULTY SUPERVISION TO AID ME IN STUDYING AND EARNING THE DEGREE WHICH I HAVE INDICATED ON THIS APPLICATION. FURTHERMORE, I FULLY UNDERSTAND WHEN I SIGN THIS APPLICATION I AM CONTRACTING TO PAY THE AMOUNT INDICATED ABOVE. I AM IN FULL AGREEMENT AND WILL ABIDE BY THE SAME. MY NAME IN THE BOX BELOW WILL ACT AS MY LEGAL SIGNATURE.
